You must be a resident of the State of Ohio to qualify.
Medical records from an Ohio MD or DO are required for service.


If you have ANY condition that results in pain that is chronic & severe or intractable, or ANY condition that causes seizures, then your condition likely qualifies.  


In addition, the following named conditions qualify:


  • Pain that is chronic and severe 
  • Pain that is Intractable
  • Acquired immune deficiency syndrome 
  • Alzheimer's disease 
  • Amyotrophic lateral sclerosis 
  • Cancer 
  • Chronic traumatic encephalopathy 
  • Crohn's disease 
  • Epilepsy or another seizure disorder 
  • Fibromyalgia 
  • Glaucoma 
  • Hepatitis C 
  • Inflammatory bowel disease 
  • Multiple sclerosis​
  • Parkinson's disease 
  • Positive status for HIV 
  • Post-traumatic stress disorder
  • Sickle cell anemia
  • Spinal cord disease or injury 
  • Tourette's syndrome 
  • Traumatic brain injury 
  • Ulcerative colitis

Of course, a physician will need to look at other treatments or conditions surrounding your overall health prior to recommending cannabis as a medical treatment.
